Liability Waiver & Consent
This will serve to certify that I volunteer to participate in the activities of VetsBoats.
Activities will involve risks including dangers of serious injury, permanent disability, death, and social economic loss.
Activities will involve participation and supervision of others, without whom activities could not occur, and whose actions may result in alteration of perceived risk;
My general state of health allows participation in physical activities that may be strenuous;
I will give consideration to my capabilities and knowledge before participating in any activity;
Should I observe a condition I consider unacceptably hazardous or dangerous, or believe I am not qualified to participate, I will not participate in that activity and notify my supervisor.
I will maintain health insurance with a generally recognized provider;
I hereby release from all liability VetsBoats, its agents or members from liabilities, claims, losses, or damages on my account resulting from negligent acts, including rescue or first aid, during my participation in activities;
I will indemnify and hold harmless VetsBoats, its agents or members for expenses of litigation, attorney fees, loss, damage, or other costs that may result from such claim should others on my behalf make a claim against VetsBoats its agents or members; and,
I authorize appropriate emergency medical attention should I become severely injured or physically incapacitated during my participation in activities.
My signature indicates that I have read and understand this agreement and understand that it includes the relinquishing of significant rights. I also affirm that I have answered the questions on this application truthfully. I have signed this freely and intend it to be a complete and unconditional assumption of risk and release of liability to the fullest extent allowed by law. I agree that if any portion of this agreement is held to be invalid by a court of law the balance shall continue in effect.
